The Department of State says you should have your passport, travel itinerary if you have already made arrangements, dates of your last five U.S. visits if applicable, and potentially your international travel history for the previous five years. You may also need education and work-history information from a résumé or CV.
Some visa categories need extra information
F, J and M applicants should have the SEVIS ID and school or program address from the I-20 or DS-2019. Petition-based temporary workers should have a copy of the I-129 available. Other temporary workers may be asked for employer information.

The Department of State warns applicants to answer DS-160 questions accurately and completely. Treat your supporting documents as references: reconcile dates, names, addresses and travel history before submitting instead of rushing through the form.
